POSITION SUMMARY

The Care Navigator is responsible for delivering person-centered care navigation support within a community-based organization to frail, home-bound seniors. The Care Navigator will work collaboratively with a project team that includes researchers from the West Health Institute and key staff from Meals on Wheels San Diego County to implement an innovative pilot that enhances home delivered meal services by standardizing and responding to health and safety screening assessments that are conducted by drivers who interact with meal recipients on a daily basis. The Care Navigator, in collaboration with the project team will develop and implement a work flow and decision tree that triages and responds to daily safety and wellness reports received from the drivers. Communication pathways will be established to document, track and report meal recipients’ needs and concerns with the individual’s emergency contact, medical and non-medical providers. The position is based at Meals on Wheels San Diego County in San Diego, California with occasional travel to the West Health Institute in La Jolla, California.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Work collaboratively with project team to ensure the successful implementation of pilot
  • Coordinate and deliver care navigation support that is timely, effective, efficient, equitable and person-centered
  • Develop effective working relationships with drivers, meal recipients and their caregivers, and meal recipients’ medical and non-medical providers
  • Help plan, implement, and maintain data collection of drivers observations as reported in health and safety screening assessment and actions taken in response to reports
  • Help develop and implement a decision tree or algorithm to tailor response to observations reported in the health and safety screening assessment
  • Manage all information and data within the scope of the position with complete confidentiality and security
  • Maintain confidentiality of meal recipients health and safety information
  • Assist with the development and implementation of an electronic data collection tool and enter data into tool as needed
  • Compile and report project-related information and data for summary reports for various stakeholders
  • Document all correspondence and communication pertinent to the pilot
  • Coordinate with research team to provide assistance as needed
  • Perform other job-related duties as assigned
  • Travel as needed to support research activities

QUALIFICATIONS, SPECIAL SKILLS, AND EDUCATION

  • Minimum Bachelor’s degree in Social Work, Social Services or Gerontology, or related field
  • Minimum of 2 years Social Services experience working with seniors and connecting seniors to medical and non-medical support services
  • Experience working with healthcare professionals a plus
  • Solid working knowledge of programs and services available in San Diego County for seniors
  • Experience working with community-based organizations
  • Must have excellent communication, written, presentation and interpersonal skills
  • Ability to interact and communicate effectively with project team, meal recipients, volunteers and collaborators
  • Ability to work independently and as part of a team
  • Good problem solving skills and critical thinking skills required
  • Data entry and computer literacy required
  • Ability to work with other forms of technology (e.g., software applications, database tools, mobile technology) preferred
  • Must be organized, attentive to detail, and possess a friendly and professional demeanor
  • Desire to make a difference in the lives of seniors
  • Valid driver’s license, clean DMV record and insurance required
